Is There Anyway to Cure Nasal Polyps without Surgery ?

Chronic rhinosinusitis with nasal polyposis ( nose polyps ) is a numbrella term for a heterogeneous group of inflammatory disorders characterized by persistent polypoid inflammation ofthe sino nasal mucosa (>_12 weeks) and nasal obstruction. Symptoms are often severe and only partially responsive to treatment, and disease is commonly associated with difficult-to-treat asthma. There is an urgent unmet clinical need to understand the immuno pathology of Nasal polyps. 

Several studies have indicated regional variation in nasal polyps endotypes. Western countries show a predominance of eosinophilic TH2-associated polyps, and Staphylococcus aureussuperantigens have beenimplicated in driving the TH2 response.

Nasal polyps reveals frequent epithelial damage, a thickened  basementmembrane, oedema, fibrotic foci and a reduced number of vessels and glands. The stroma of Nasal polyps contains abundant infiltration of inflammatory cells,predominantly activated eosinophils and increased fibro-blast numbers. Fibroblasts are the major and nearlyexclusive producers of extra-cellular matrix (ECM) proteinsin airways, including collagens, fibronectin and proteogly-cans. Increased collagen and fibronectin deposition hasbeen found in Nasal polyps. Fibroblasts alsorelease enzymes and a variety of pro-inflammatory medi-ators, including growth factors, cytokines, and chemokines which contribute to the perpetuationof inflammatory and fibrotic processes in the airways 

 Anti-inflammatory treatment with intranasal glucocorticoids (GC) has been the cornerstone of Nasal polyps therapy. The main target of GC action is the inhibition of inflam-matory cell activation and survival, although structuralcells, including fibroblasts, are also targets of GC treatment. Some patients, however, do not respond, or respond poorly, to glucocorticoids therapy, resulting in a need for repeatedsurgery to remove Nasal polyps.

To diagnose nose polps,  a fiberoptic rhinoscopic examination is essential to diagnose the Nose Polyps phenotype before initiating medical treatment.
