INCS are an effective treatment for Nasal Polyps ( Nose polyps ) but are limited in their efficacy by inconsistent nasal distribution, especially in the presence of severe nasomucosal edema or large nasal polyps. Conven¬tional nasal sprays deliver less than 5% of metered INCS into the middle meatus, the critical area of the nose for drainage of the maxillary, anterior ethmoid, and frontal sinuses. Several different delivery strategies, such as nebulization or irrigation, are used to overcome this maldistribution. To diagnose nose polps,  a fiberoptic rhinoscopic examination is essential to diagnose the Nose Polyps phenotype before initiating medical treatment. Anterior rhinoscopy only visualizes the anterior one- third of the nasal cavity and is inadequate to visualize the posterior nasal cavity and nasopharynx. Rhinoscopy is a simple, low-risk, cost-effective procedure that should be performed routinely in subjects with persistent upper airway problems. Just as office spirometry is a routine procedure to assist in the diagnosis of airflow obstruction, so too should rhinoscopy be used regularly for Nose Polyps. Nose Polyps tissues were obtained from 8 patients with nasal pol-yposis and asthma undergoing functional endoscopic sinussurgery (FESS). NM was obtained from the inferiorturbinate of 8 subjects undergoing turbin ectomy for turbinate hypertrophy with or without septoplasty for nasalseptum deviation. Chronic rhinosinusitis with nasal polyposis ( nose polyps ) is a numbrella term for a heterogeneous group of inflammatory disorders characterized by persistent polypoid inflammation ofthe sino nasal mucosa (>_12 weeks) and nasal obstruction. New study shows that fibroblasts from patients with NPs and asthma who have a poor clinical response to Glucocorticoids Nasal  treatment, display a relative resistance to the anti-proliferative effects of glucocorticoids nasal,  compared to fibroblasts from control nasal mucosa.  In addition, glucocorticoids nasal  failed to inhibit collagen expression in any fibroblast type. In contrast, GCs effectively down-regulated IL-6 and IL-8 production by nasal fibroblasts, though Nasal Polyps fibroblasts were less sensitive to dexamethasone than NM fibroblasts.  Overall, our findings reinforce the concept that glucocorticoids nasal are less effective in reducing fibrosis and remodelling of the airways than in reducing inflammation. Severe nasal polyposis can cause deformities of the nose and face. This phenomenon is called Woakes’ syndrome. The facial deformity in our patient was caused by pressure from nasal polyposis that filled the nasal cavity and paranasal sinuses.   Glucocorticoid is the first-line treatment for chronic rhinosinusitis with nasal polyps. However, the appropriate dose and the duration of treatment remain unknown. In China, nasal spray is widely used in Nasal Polyps patients, and patients with asthma or with recurrent nasal polyps will be treated with oral steroids for one to two weeks or even longer to control the symptoms.  However, an increasing number of studies have demonstrated that the regular dose of nasal spray is not enough for disease control.   Researchers divided patients into eosinophilic Nasal Polyps and non-eosinophilic Nasal Polyps based on the infiltration of eosinophils in nasal polyp tissue. In non- eosinophilic patients, the reduction in nasal polyps was similar among the three groups. The results correlated with previous studies showing that high-dose budesonide nasal nebulization could significantly reduce the NP score in eosinophilic patients16,31 and the effect of high-dose local steroid treatment was greater than that of regular nasal spray treatment. According to the results of our study, oral steroids or high-dose local steroids might be unsuitable for non- eosinophilic patients, and might increase the risk of adverse events.
