Nose polyps ( chronicrhinosinusitis with nasal polyps ) is a heterogenous mucosalinflammation involving not only the paranasal sinuses but alsothe nose. An abnormal host response to infectious microorganisms,such as viruses and bacteria, has been suggested to be responsiblefor the persistent inflammation of the sinonasal mucosa.1Although data for the prevalence of viral infection in patientswith Nose polyps are conflicting, a high frequency of viral presence wasdemonstrated in nasal epithelial cells or lavage fluid of patientswith Nose polyps.
It has been suggested that viral respiratory tractinfection influences the development or progressive courseof Nose polyps through various means.5,6In vitrostudies demonstratedthat VRIs can damage the epithelial cells covering the sinonasalcavity, resulting in dysfunction of the epithelial barrier andincreased bacterial adhesion.
However, few studies have evaluatedwhether an apparent deficiency in antiviral immunity or the interferon response might represent a deficient innate immuneresponse in patients with Nose Polyps .The interferon response to VRIs occurs through patternrecognition receptors and Toll-like receptors. Glucocorticoids (oral or intranasal) and macrolide antibiotictreatment are recommended for patients with Nose Polyps .Despitethe known beneficial clinical effects of glucocorticoids,glucocorticoid treatment is also known to enhance viralreplication and is associated with prolonged viral isolation innasal washes during naturally occurring rhinovirus infection.Macrolides exhibit antimicrobial activity in addition toimmunomodulatory properties and inhibit the synthesis orsecretion of inflammatory cytokines.
